MEDICAL RECORDS DESTRUCTION NOTICE
If you have ever been treated at the Carraway
Methodist Medical Center at any point during
that institution's existence, you should be aware that
a substantial portion of medical records
have been abandoned on the premises and are soon to
be destroyed pursuant to an Order of the
Jefferson County Circuit Court.
Any and all paper medical records, x-rays, and/or pathology
slides remaining on site at Carraway
Methodist Medical Center will be destroyed commencing
no earlier than July 15, 2010. The
only paper medical records being destroyed are those
which were created through 1989.
All x-rays andlor pathology slides, regardless of date,
are being destroyed. If you are
interested in obtaining copies of your medical records
that are the subject of this Destruction
Notice, then you must timely send a written request
to the following address, specifically
identifying the exact medical records you are requesting
by patient name, social security number,
name of physician, and/or other identifying factors,
as required by applicable federal and state
laws:

WRITTEN REQUESTS MUST BE RECEIVED ON OR BEFORE
JUNE 30, 2010. Any
person or entity who makes a timely written request
will be notified of the day and time to
retrieve the requested records. To the extent
that any requested medical records can be located,
any and all requests for medical records will be honored
for a retrieval and copying fee of
$50.00, payable in cash or check at the time of retrieval.
Records created after 1989 will remain available by
following the instructions found in the
"Medical Records Information" section below
or by calling the Montgomery Office of Southern Records
Management
Corp. at 334-394-2355 and asking for the Carraway representative.
MEDICAL
RECORDS INFORMATION
Southern Records
Management Corp, acting as an agent of Physicians Medical
Center Carraway, will continue to provide photocopies
of medical records to requesting parties for a period
of time.
All request letters, subpoenas, etc., must be addressed
to Physicians Medical Center Carraway, 1600 Carraway
Boulevard, Birmingham, AL 35234 and mailed or delivered
to:

A valid, HIPAA compliant authorization (attached to
this web page) and a copy of a Federal or State issued
photo identification in the name of the requestor must
be attached to the authorization form. (Attorneys, Federal
and State agencies do not need to include a copy of
the photo identification.) Please fill in all blanks
or the form will be returned to you for completion before
records will bereleased. Be specific when requesting
records. Specify the date or date range of the records
needed as well as the parts of the record needed. (EXAMPLE:
Do not request the entire record when all you need is
the discharge summary, history and physical and operative
report.)
The cost of obtaining records is:
$5.00 --- Retrieval Fee
$1.00 --- Per Page for Pages 1 – 25
$0.50 --- Per Page for Pages 26+
Postage and Handling
10% Applicable Sales Tax for in State requests unless
exempt
Pre-payment will be required for all requests unless
other arrangements are made in advance. You will be
mailed or faxed an invoice prior to release of records.
Records will be released upon receipt of payment. Credit
card payments will be accepted after April 13th. Please
check the website at that time for details.

ATTENTION:
PMCC EMPLOYEES
Blue Cross had worked out a
plan for employees that were employed at the time of
the hospital closing to be able to continue their insurance
coverage. Click

EDUCATION
OPPORTUNITIES
All dislocated workers qualify for tuitioin at 50%
off
at Jefferson State Community College and
Lawson State Community College.

EMPLOYMENT
VERIFICATION
Physicians Medical Center has entered into a business
agreement with Employment Screening Services (ESS)
to provide employment verification information on
individuals which were employed by Physicians Medical
Center and Carraway Medical Center. ESS is a national
background screening company which Inc. 5000 has ranked
among the fastest-growing private companies in the
country for the second straight year.
Request for employment verifications require a hand
signed release from the applicant and a fee of $8.50.
