180 day elimination period. Premium is based on rate schedule according to employee’s age for each $100 of monthly base salary.

Group Life Insurance

One times base annual salary paid by Physicians Medical Center Carraway up to a Maximum of $50,000
Employee may elect an additional amount up to $100,000 without evidence of Insurability. Minimum of $10,000 and maximum of 5 time employees base annual salary, if approved with EOI. $300,000 maximum coverage available.
Spousal Life Insurance and Dependent Life Insurance
Full time
Becomes effective after 6 months of F/T service. Must make election within 30 days of employment
Employee may elect spousal coverage of up to $30,000 or 50% of coverage elected by the employee. Employee may elect up to $10,000 coverage on each dependent child for up to $2.00 per month
Employee may apply for additional coverage on their spouse of up to 50% of employee’s elected amount, If approved with EOI. Maximum coverage of $150,000. Employee must elect voluntary coverage to be eligible for dependent children coverage or spousal coverage.
Flexible Benefit Plan

Employees may also choose to pay expenses for dependent care and medical expenses with pre-tax dollars. Must meet qualifications.
